Incident Reporting Designed for Speed and Simplicity
When an incident occurs, capture and forward the most critical elements of an incident as quickly as possible. For most incidents, the key pieces of information are things like:
- What happened?
- How bad is it?
- When did it happen?
- Where did it happen?
- Additional comments
- Optional photos or attachments

Immediate Notifications
Once an Incident Report form is submitted, you and your managing group can receive instant email and/or push-notifications so you can immediately know what happened and begin taking the necessary actions.
Near Miss Reporting
Report on near misses to identify potential hazards or incidents that did not result in any personal injury or property damage. A “near miss” is an unplanned event that had the potential to cause injury or damage but did not. Near miss reports also provide supervisors with information that allows the hazard to be resolved before it ever becomes an incident.
Encouraging Worker Safety
Near Miss Reporting is one part of a hazard avoidance strategy called Behavior-Based Safety. Behavior-Based Safety (often called BBS) encourages workers to be more “safety aware” by constantly searching for safety hazards (through observation reporting) and by identifying and addressing near misses before they become incidents.
